Read the article, loved it, and I think you covered the issue of clinical notes very well (and the fact that you still managed to inject a bit of humour into it is a feat all in itself!) It seems like a straightforward task writing clinical notes but you've shown how easy a small error or lack of detail can lead to big mistakes or at least an embarrassing amount of confusion.
